Our qualified physicians and clinical staff understand your concerns and the needs of your family because we also live in your community and experience the same healthcare concerns.      

  • We are a Federally Qualified Healthcare Center which means we accept most insurance, including Medicaid and Medicare, and offer sliding fee scale charges to those who qualify.   Please see our sliding scale brochure.

  • We strive to make each visit as easy as possible and offer a pleasant, friendly environment where you and your family feel comfortable in your healthcare journey.   

  • We address all your non-hospital primary healthcare needs, while also offering supportive programming to address long-term wellness concerns such as weight control, diabetes, cardiovascular health, general nutrition, and comprehensive wellness.  

  • We encourage appointments, but welcome walk-ins. We have an on-site pharmacy and offer on-site laboratory services for your convenience.   

  • We are conveniently located near I-75 on Alabama Highway with plenty of parking.

For patients that are uninsured or choose not to file insurance, TMFC offers eligibility for the sliding scale program. Under the sliding scale program, TMFC offers discounts off our regular charges based on a person’s income and the number of people in the household. When you visit TMFC our staff will help you determine the correct payment group

*Good Faith Estimates require providers and facilities to provide good faith estimates of charges for care to uninsured or self-pay individuals upon scheduling care or on request, and for individuals with certain types of coverage, to submit good faith estimates to the individual’s plan or issuer. FQHCs must have a patient-provider dispute resolution process for uninsured (or self-pay) individuals to contest charges that are “substantially in excess” of the good faith estimate.

Tri-Med Family Care is honored to be a Federally Qualified Health Center, and awarded recognition by the National Committee for Quality Assurance as NCQA’s Patient Center Medical Home Program (PCMH). Our community, our families, our privilege in providing accessible quality, affordable healthcare since 2009. Although our name has changed, our excellent service has not. We are still your hometown primary healthcare provider who cares about our patients like family.

 
 
  • In addition to detecting diseases and other chronic conditions, an annual exam and other preventive care can help you maintain healthy habits like physical activity and a well-rounded diet. It can also help you establish a routine and rapport with your primary care physician.

  • An annual physical is a crucial form of preventive care. Even if you feel perfectly healthy, your healthcare provider will run routine lab work and other diagnostic tests that can serve as valuable indicators for irregularities you wouldn’t be able to spot otherwise.

  • If you’re transitioning from an acute care setting or emergency room visit, you’ll receive hospital follow-up with your TRI-MED primary care provider. These visits allow our providers to review medication changes and test results following hospital and other acute care encounters, as well as provide a chance to evaluate new treatment options.

  • TRI-MED provides annual well-woman preventive care for adult women (age and developmentally appropriate), including cervical cancer screening, preconception care, and many services necessary for prenatal care. Other prenatal care and screening services necessary for optimal women’s health are referred to specialty providers.

  • As a Georgia Family Planning System (GFPS) family planning service site, TRI-MED provides access to high-quality pregnancy planning and prevention services integrated with other preventive health care for women, men, and adolescents.
